چکیده

Carcinoid is a rare tumor originating from neuroendocrine cells, which are generally localized in the gastrointestinal tract. It has an annual incidence of 1.5 persons per 100 000. Usually, only carcinoid tumors that invade the liver result in pathological changes to the heart. The cardiac manifestations are caused by paraneoplastic effects of vasoactive substances such as serotonin, bradykinin, tachykinins, histamine, and prostaglandins that are released by the malignant cells, rather than by direct cardiac metastatic involvement. Carcinoid heart disease is manifested by structural changes in the right heart secondary to severe fi brotic endocardial plaques resulting from elevated blood concentrations of these vasoactive substances. Therefore, carcinoid heart disease is a rare but important cause of intrinsic tricuspid and pulmonary valve disease, leading to signifi cant morbidity and mortality secondary to right heart failure. Mitral or aortic valve dysfunction is less frequent and presents usually as regurgitation. Because the vasoactive substances that are produced by the tumor are metabolized in the lungs, the presence of left heart disease usually refl ects the presence of a persistent foramen Received: 27 January 2011 © The Japanese Association for Thoracic Surgery 2011

Prognosis of carcinoid heart disease: analysis of 200 cases over two decades.

BACKGROUND The long-term prognosis of patients who develop carcinoid heart disease and the effect of cardiac surgery on outcome are not well established. METHODS AND RESULTS In this retrospective study, we identified 200 patients with carcinoid syndrome referred for echocardiography in whom the diagnosis of carcinoid heart disease was confirmed. Patients were divided into 3 groups of similar ...

Metastatic carcinoid tumor to the heart: echocardiographic-pathologic study of 11 patients.

OBJECTIVE We sought to investigate the clinical and echocardiographic (echo) characteristics of metastatic carcinoid tumor in the heart. BACKGROUND Right-sided valvular dysfunction is the hallmark of carcinoid heart disease. Cardiac metastases are uncommon in carcinoid syndrome. Features of patients with metastatic carcinoid tumor involving the heart (MCH) have not been well described. METH...

Carcinoid Heart Disease: Starting From Heart Failure

Carcinoid syndrome is the constellation of symptoms mediated by humoral factors produced by some carcinoid tumors. It consists primarily of vasomotor symptoms, gastrointestinal hypermotility, hypotension, and bronchospasm, due to the production and release of vasoactive substances. Carcinoid heart disease occurs in more than 50% of patients with carcinoid syndrome; in some cases, it represents ...
